PH PHAmily Reunion T-Shirt Order Form

 All members and guests who will be attending the Prince Hall PHAmily Weekend activities are encouraged to purchase one of our custom-designed SC Prince Hall PHAmily Reunion t-shirts for $25.00 each. The shirts will come in Black and/or Red.  In order to keep a better track of the shirts, all members are asked to order their shirts through their Lodge and/or Chapter, if possible.  Please see the attached flyer with all of the pertinent shirt information.  There is also a separate T-Shirt Order Form that you are asked to use to order your t-shirts.  Both the flyer and order forms have all payment information at the bottom.  If you have questions about the shirts, please feel free to reach out to Sis.
